Health Services

If a student becomes ill or injured at school, he/she must report the illness or injury to a teacher, the nurse or to the office.  No student is to leave school because he/she is ill without first securing permission. A parent excuse will be used for students that arrive at the Clinic before school starts. 

 Your student must be fever free, without medication, for 24 hours before returning to school.

25-26 Immunizations

The State of Indiana requires that each student entering 6th grade receive a Meningitis Vaccine and a TDaP Vaccine before the 1st day of school.  Any student not meeting this requirement will receive one notice; after 20 days, the student may be excluded from school until we receive notifications of immunizations.  Exemptions to the immunization requirements shall be granted only for medical, religious or other reasons allowed by the Indiana State law.

     Below are the School Immunizations requirements:

  • 3 Hepatitis B

  • 5 DTap (Diphtheria, Tetanus & Pertussis

  • 4 Polio (Inactivated Polio)

  • 2 MMR (Measles, Mumps, Rubella)

  • 2 Varicella 

  • 1 Tdap (Tetanus & Pertussis)

  • 1 MCV4 (Meningoccal Conjugate) 

  • 2 Hepatitis A 

Medication at School

  Medications must be brought to the school clinic immediately upon arrival to school and will be stored in a locked cabinet.   Medication must be current and in the original container. 

 Students are not permitted to transport medications home on the bus. If a student brings medication to the school, it is to be brought to the clinic immediately.

 * A parent/guardian signed permission slip must be on file at school in order for medication to be dispensed to your child. 

Medical Conditions

A doctor's order or care plan is required for any student with a known medical condition (i.e. heart condition, seizure disorder, diabetes, asthma, allergic reactions, or other illness requiring doctor's care, including recent surgery, etc.). A written doctor's order explaining activity restrictions will be kept on file for the school year. This must be updated yearly. A copy will also be given to the NMS Guidance Department so that class schedules may be adjusted accordingly.

 ALL students will participate in P.E. and/or Aquatics classes as scheduled unless a doctor’s order is on file listing restrictions.
